How Can I Get Care In My Home?

 

Step 1: Your family Doctor, Nurse Specialist, or Nurse Practitioner can refer you to Geriatric nurse for assessment/needs assessor for in-home service. If you qualify, they will take it from there.

Step 2: You can contact Ministry of Health Disability and Support Link assessed Carer Support Days maybe used to subsidize our daily fees.

Step 3: A Geriatric nurse will call and set a time to assess your needs in your home. (Keep in mind this is a service for recipients and long term that need hands-on personal care, not simply housekeeping, laundry or meal preparation)

Step 4: We’d love to be your home care agency of choice. You will be given a list of home care agencies and asked to choose one.
